During my first decade of physiatric practice, my tools for addressing spasticity and muscle hypertonicity were limited to casting/splinting, phenol injections, and surgery. The availability of neurotoxins has truly altered the approach to the management of spasticity (as well as bestowing youthfulness upon many a brow that previously was furrowed). While phenol neurolysis can play a meaningful role in preserving joint movement in spasticity, neurotoxins have permitted a nuanced and focal approach to balancing muscle forces.
