I was a little skeptical of how well this would work, as my kids are notoriously resistant to actually applying any of the self-regulating techniques they know when they are upset. But we started using this book a few weeks ago and had our first really explosive incident since then the other day. I got the book and opened it to some of the pages we had already done, and my son grudgingly repeated the exercises on them (something he would NEVER have done it someone simply told him to do a breathing exercise). And... it worked. It wasn't perfect, but he calmed down much, much faster than usual. I am more optimistic that he's actually retaining some of what's in the book now, and that I might be able to help him out it into regular practice.
