In addition, each incident is presented in sufficient detail to gain an understanding of root causes for the event, the impact the incident had, and focuses on lessons learned for improving process safety risk reduction efforts. The incidents are grouped by incident type, including: reactive chemicals; fires; explosions; environmental/toxic releases; and transportation incidents. The book also covers incidents from non-process-related industries that illustrate similar weaknesses in the basic process safety management elements.
